Planning for our loved ones after we are gone

The Letter of Intent is a living document you create that passes vital information about a person with a disability to future caregivers. It should include things about your loved ones including where they live, employment, medical needs, interests and other important aspects of their daily routine along with their goals for their future. This document is a work in progress and needs to be updated regularly.

The hardest part is getting started! We use Google docs and a couple of template samples to work from. You can join us if you are just starting or need the company and a set time to do this project.
