If you want some good company and guidance, join our monthly parent group to help you navigate this big transition. With the support of other parents and a proven approach, parents can align with their teen to co-create a great life.

Develop and implement your own action plan with this three-part approach:

• Learn about and begin to access adult services and funding

• Identify and address home and community based skills teens need to achieve long-term goals

• Utilize the transition goals in the IEP to align with adult life
